Most activities that happen outside of youth night need either ward or stake approval. At the ward level, the bishopric has the responsibility for approving activities. At the stake level, the stake presidency has that responsibility. Use these forms and resources for dotting all your i's and crossing all your t's in this approval process.

When should I use this form? If you are planning a ward or branch event or outing that includes an overnight stay or travel inside the local area, complete this form and submit it to your bishop two or three weeks prior to advertising or promoting the event.
If you are planning an event or outing that involves one or more high risk activities or that requires travel outside the local area, stake approval is required. When seeking stake presidency approval, submit the following:
Event and Activity Plan
Activity Safety Plan
Event Itinerary (only for activities two or more days in length)
Email this information to Josh Schmidt, our Stake Safety Specialist (jschmidt01@atsu.edu), two or three weeks prior to advertising or promoting the event.
When should I use this form? For all activities involving one or more of the high-risk activities (aquatics; firearms; climbing, rappelling, or caving; off-road vehicles; archery; or horseback riding) or that require travel outside the local area, an Activity Safety Plan is required.
Submit the Activity Safety Plan with your Event and Activity Plan two to three weeks prior to advertising or promoting the activity.
When should I use this form?
Ward Application: Use this form for all activities and outings that take place outside of your ward boundaries, for all overnight activities, and for ward activities with higher than normal risks. Present form to ward youth leaders.
Stake Application: Use this form for all stake activities and outings that take place outside of our stake boundaries, involve an overnight stay, or for stake activities with higher than normal risks. Present form to stake youth leaders.

Protecting Children and Youth
All leaders and teachers of children or youth are to complete this training within one month of being called. The training must be repeated once every three years. Follow this LINK to access the training.
